ABSTRACT Objective: Recent studies have shown a relationship between adipose tissue and coronary artery disease (CAD). The ABCA1 transporter regulates cellular cholesterol content and reverses cholesterol transport. The aim of this study was to determine the relationship between single nucleotide polymorphisms (SNPs) R230C, C-17G, and C-69T and their expression in epicardial and mediastinal adipose tissue in Mexican patients with CAD. Subjects and methods: The study included 71 patients with CAD and a control group consisting of 64 patients who underwent heart valve replacement. SNPs were determined using TaqMan probes. mRNA was extracted using TriPure Isolation from epicardial and mediastinal adipose tissue. Quantification and expression analyses were done using RT-qPCR. Results: R230C showed a higher frequency of the GG genotype in the CAD group (70.4%) than the control group (57.8%) [OR 0.34, 95% CI (0.14-0.82) p = 0.014]. Similarly, C-17G (rs2740483) showed a statistically significant difference in the CC genotype in the CAD group (63.3%) in comparison to the controls (28.1%) [OR 4.42, 95% CI (2.13-9.16), p = 0.001]. mRNA expression in SNP R230C showed statistically significant overexpression in the AA genotype compared to the GG genotype in CAD patients [11.01 (4.31-15.24) vs. 3.86 (2.47-12.50), p = 0.015]. Conclusion: The results suggest that the GG genotype of R230C and CC genotype of C-17G are strongly associated with the development of CAD in Mexican patients. In addition, under-expression of mRNA in the GG genotype in R230C is associated with patients undergoing revascularization.

ABSTRACT

Objetivo: Traçar um perfil epidemiológico do medo do parto em gestantes em Santos, correlacionando as variáveis idade, escolaridade, estado civil, paridade, perdas gestacionais prévias e intercorrências gestacionais. Métodos: Estudo transversal, realizado em programa pré-natal de risco habitual em cinco Unidades Básicas de Saúde de Santos, com coleta de dados entre novembro de 2019 e fevereiro de 2020. Critérios de inclusão: idade gestacional maior ou igual a 28 semanas, maiores de 18 anos, preferência por parto vaginal, alfabetizadas em português. A amostra foi de 67 participantes, que responderam ao Questionário sobre o Medo Percebido do Parto (QMPP), versão portuguesa do Wijma Delivery Expectancy/Experience Questionnaire versão A, e a uma ficha com dados sociodemográficos. Resultados: A pontuação média no QMPP foi 79,3. Observou-se uma prevalência de 31,4% de gestantes com escore maior ou igual a 85, que compreende medo do parto intenso e tocofobia. A pontuação no QMPP apresentou correlação fraca positiva com idade. Conclusão: A prevalência de medo do parto no presente estudo se assemelhou àquela observada em metanálises internacionais. Este estudo pode embasar tanto futuras pesquisas sobre medo do parto no Brasil como projetos públicos para diagnóstico e tratamento dessa condição nas gestantes brasileiras.(AU)


Objective: To obtain an epidemiologic profile of the pregnancies affected by fear of childbirth in Santos, correlating variables such as: age, schooling, marriage status, parity, previous gestational losses, and pregnancy complications. Methods: Transversal study located in five polyclinics in Santos, in habitual risk prenatal program. The data was collected between November 2019 and February 2020. Inclusion criteria: gestational age equal or greater than 28 weeks, women older than 18 years, wish for vaginal birth, alphabetized in Portuguese. The final sample was 67 participants, which completed the Questionário sobre o Medo Percebido do Parto, Portuguese version of Wijma Delivery Expectancy/Experience Questionnaire version A, and a sheet with sociodemographic data. Results: The average score on QMPP was 79,3. It was found a prevalence of 31,4% of pregnant women with scores equal or greater than 85, which comprehends intense fear of childbirth and tocophobia. The QMPP score presented a weak positive correlation with age. Conclusion: The prevalence of fear of childbirth obtained in this study resembles the results of international metanalysis. This study can base future research about fear of childbirth in Brazil, as well as public projects to diagnose and treat this condition in Brazilian women.(AU)

Abstract The effects of the non-steroidal anti-inflammatory drugs (NSAIDs) on bone quantity and quality were investigated for years. However, there is lack of information on the impact of NSAIDs on the quality of tooth-supporting alveolar bone in absence of periodontal inflammation. Thus, the aim of this study was to evaluate histometrically the influence of a selective COX-2 NSAID (Meloxicam) on the inter-radicular bone mineral density in rats. Forty-nine adult male Wistar rats were randomly divided into four experimental groups: Subcutaneous injection of 0.9% sterile saline for 15 days (G1; n=12) and 45 days (G2; n=11); and subcutaneous injection of Meloxicam for 15 days (G3; n=13) and 45 days (G4; n=13). Mineral density was histometrically determined in the inter-radicular area of the 1st mandibular molars and data analysis performed by two-way ANOVA (a=5%). Results showed no interaction between time and treatment (p>0.05) and that meloxicam did not affect the alveolar bone density. In contrast, it was found that inter-radicular alveolar bone density increased with time (91.88±3.08% and 92.86±2.38% for groups 15 and 45 days, respectively) (p<0.05). Within the limits of this study, daily administration of a selective COX-2 inhibitor (Meloxicam) did not affect the quality of the inter-radicular alveolar bone in absence of periodontal infection.


Resumo Os efeitos dos fármacos anti-inflamatórios não esteroidais (AINEs) sobre a quantidade e qualidade óssea tem sido investigados ao longo dos anos.Entretanto, há falta de informação sobre o impacto dos AINEs na qualidade do osso alveolar de suporte na ausência de inflamação periodontal. Assim, o objetivo deste estudo foi avaliar, histometricamente, a influência de um AINE seletivo para COX-2 (Meloxicam) na densidade mineral óssea inter-radicular em ratos. Quarenta e nove ratos Wistar, machos e adultos foram divididos aleatoriamente em quatro grupos experimentais: injeções subcutâneas de 0,9% de solução salina estéril por 15 dias (G1, n=12) e 45 dias (G2, n=11); e injeções subcutâneas de Meloxicam por 15 (G3, n=13) e 45 dias (G4, n=13). A densidade mineral foi determinada histometricamente na área inter-radicular dos primeiros molares mandibulares e a análise dos dados realizada por meio de ANOVA (a=5%). Os resultados mostraram nenhuma interação entre tempo e tratamento (p>0,05) e que o meloxicam não afetou a densidade óssea alveolar. Em contraste, foi encontrado que a densidade óssea alveolar inter-radicular aumentou ao longo do tempo (91,88±3,08% e 92,86±2,38% para os grupos 15 e 45 dias, respectivamente) (p<0,05). Dentro dos limites deste estudo, a administração diária de um inibidor seletivo para COX-2 (Meloxicam) não afetou a qualidade do osso alveolar inter-radicular na ausência de infecção periodontal.

ABSTRACT

Contexto: A doença de Gaucher (DG) é uma doença lisossômica de depósito, presente com maior prevalência entre judeus asquenazes, e está relacionada à mutação N370S. Apresenta-se clinicamente de três formas: não neuropática ou tipo 1, mais prevalente e com bom prognóstico; neuropática aguda ou tipo 2; neuropática crônica ou tipo 3. No tratamento, emprega-se o imiglucerase, um repositor enzimático. Descrição do caso: Paciente com 20 anos, primigesta, nulípara, com 36 semanas de idade gestacional, diagnosticada com DG tipo I aos oito anos, foi internada para acompanhamento de sua afecção de base, controle da pré-eclâmpsia e infecção do trato urinário. Devido à variação dos índices hematimétricos, foi induzido o parto com 37 semanas e 2 dias. O recém-nascido não apresentou quaisquer distúrbios e seguiu em alojamento conjunto. Discussão: Observou-se a necessidade de controle clínico ativo, mesmo com a paciente em uso regular do imiglucerase, evitando repercussões graves para o binômio materno-fetal. Conclusões: Salienta-se a necessidade de controle hematológico ativo, via de parto conforme indicação obstétrica, seja via alta ou baixa, e níveis plaquetários superiores a 50.000/mm³.

ABSTRACT

Inherited endocrine tumors have been increasingly recognized in clinical practice, although some difficulties still exist in differentiating these conditions from their sporadic endocrine tumor counterparts. Here, we list the 12 main topics that could add helpful information and clues for performing an early differential diagnosis to distinguish between these conditions. The early diagnosis of patients with inherited endocrine tumors may be performed either clinically or by mutation analysis in at-risk individuals. Early detection usually has a large impact in tumor management, allowing preventive clinical or surgical therapy in most cases. Advice for the clinical and surgical management of inherited endocrine tumors is also discussed. In addition, recent clinical and genetic advances for 17 different forms of inherited endocrine tumors are briefly reviewed.

OBJECTIVE: To evaluate frequency, anatomic presentation, and quantities of supernumerary parathyroids glands in patients with primary hyperparathyroidism (HPT1) associated with multiple endocrine neoplasia type 1 (MEN1), as well as the importance of thymectomy, and the benefits of localizing examinations for those glands. METHODS: Forty-one patients with hyperparathyroidism associated with MEN1 who underwent parathyroidectomy between 1997 and 2007 were retrospectively studied. The location and number of supernumerary parathyroids were reviewed, as well as whether cervical ultrasound and parathyroid SESTAMIBI scan (MIBI) were useful diagnostic tools. RESULTS: In five patients (12.2%) a supernumerary gland was identified. In three of these cases (40%), the glands were near the thyroid gland and were found during the procedure. None of the imaging examinations were able to detect supernumerary parathyroids. In one case, only the pathologic examination could find a microscopic fifth gland in the thymus. In the last case, the supernumerary gland was resected through a sternotomy after a recurrence of hyperparathyroidism, ten years after the initial four-gland parathyroidectomy without thymectomy. MIBI was capable of detecting this gland, but only in the recurrent setting. Cervical ultrasound did not detect any supernumerary glands. CONCLUSION: The frequency of supernumerary parathyroid gland in the HPT1/MEN1 patients studied (12.2%) was significant. Surgeons should be aware of the need to search for supernumerary glands during neck exploration, besides the thymus. Imaging examinations were not useful in the pre-surgical location of these glands, and one case presented a recurrence of hyperparathyroidism.


OBJETIVO: Avaliação da frequência, da localização anatômica e do número de paratireoides extranumerárias em pacientes com hiperparatireoidismo primário (HPT1) associado a neoplasia endócrina múltipla tipo 1(NEM1), além da avaliação da importância da timectomia e da utilidade dos exames radiológicos para localização destes. MÉTODOS: Foram avaliados de forma retrospectiva 41 pacientes portadores de NEM1 com HPT1 submetidos a paratireoidectomia entre 1997 e 2007. O número de glândulas supranumerárias encontradas e a sua localização foram revisados, assim como a utilidade do ultrassom cervical e do SESTAMIBI (MIBI) de paratireoide como ferramentas diagnósticas. RESULTADOS: Em cinco pacientes (12,2%) foram identificadas glândulas supranumerárias. Em três destes (40%), as glândulas estavam próximas à glândula tireoide e foram encontradas durante a exploração cirúrgica. Os exames de imagem não foram úteis para a localização destas glândulas. Em um caso, apenas o exame anatomopatológico foi capaz de encontrar uma glândula extranumerária microscópica localizada no timo. No último caso, uma quinta glândula foi ressecada por meio de esternotomia após a recidiva do hiperparatireoidismo, cerca de 10 anos após a paratireoidectomia realizada sem timectomia na ocasião. Neste caso o MIBI detectou esta paratireoide apenas após a recidiva da doença. Em nenhum dos casos o ultrassom cervical foi capaz de detectar glândulas extranumerárias. CONCLUSÃO: A frequência de paratireoides supranumerárias em nossa casuística foi significativa (12,2%). Durante a exploração cervical, o cirurgião deve estar atento para localizar glândulas extranumerárias além do timo. Exames de imagem não foram úteis na localização préoperatória dessas glândulas, e em um caso houve recidiva do hiperparatireoidismo.

OBJECTIVE: To investigate a possible direct, growth hormone-releasing, hormone-independent action of a growth hormone secretagogue, GHRP-2, in pituitary somatotroph cells in the presence of inactive growth hormonereleasing hormone receptors. MATERIALS AND METHODS: The responses of serum growth hormone to acutely injected growth hormone-releasing P-2 in lit/litmice, which represent a model of GH deficiency arising frommutated growth hormone-releasing hormonereceptors, were compared to those observed in the heterozygous (lit/+) littermates and wild-type (+/+) C57BL/6J mice. RESULTS: After the administration of 10 mcg of growth hormone-releasing P-2 to lit/lit mice, a growth hormone release of 9.3±1.5 ng/ml was observed compared with 1.04±1.15 ng/ml in controls (p<0.001). In comparison, an intermediate growth hormone release of 34.5±9.7 ng/ml and a higher growth hormone release of 163±46 ng/ml were induced in the lit/+ mice and wild-type mice, respectively. Thus, GHRP-2 stimulated growth hormone in the lit/lit mice, and the release of growth hormone in vivo may be only partially dependent on growth hormone-releasing hormone. Additionally, the plasma leptin and ghrelin levels were evaluated in the lit/lit mice under basal and stimulated conditions. CONCLUSIONS: Here, we have demonstrated that lit/lit mice, which harbor a germline mutation in the Growth hormone-releasing hormone gene, maintain a limited but statistically significant growth hormone elevation after exogenous stimulation with GHRP-2. The present data probably reflect a direct, growth hormone-independent effect on Growth hormone S (ghrelin) stimulation in the remaining pituitary somatotrophs of little mice that is mediated by growth hormone S-R 1a.

ABSTRACT

The finished version of the human genome sequence was completed in 2003, and this event initiated a revolution in medical practice, which is usually referred to as the age of genomic or personalized medicine. Genomic medicine aims to be predictive, personalized, preventive, and also participative (4Ps). It offers a new approach to several pathological conditions, although its impact so far has been more evident in mendelian diseases. This article briefly reviews the potential advantages of this approach, and also some issues that may arise in the attempt to apply the accumulated knowledge from genomic medicine to clinical practice in emerging countries. The advantages of applying genomic medicine into clinical practice are obvious, enabling prediction, prevention, and early diagnosis and treatment of several genetic disorders. However, there are also some issues, such as those related to: (a) the need for approval of a law equivalent to the Genetic Information Nondiscrimination Act, which was approved in 2008 in the USA; (b) the need for private and public funding for genetics and genomics; (c) the need for development of innovative healthcare systems that may substantially cut costs (e.g. costs of periodic medical followup); (d) the need for new graduate and postgraduate curricula in which genomic medicine is emphasized; and (e) the need to adequately inform the population and possible consumers of genetic testing, with reference to the basic aspects of genomic medicine.

ABSTRACT

Hirschsprung disease is a congenital form of aganglionic megacolon that results from cristopathy. Hirschsprung disease usually occurs as a sporadic disease, although it may be associated with several inherited conditions, such as multiple endocrine neoplasia type 2. The rearranged during transfection (RET) proto-oncogene is the major susceptibility gene for Hirschsprung disease, and germline mutations in RET have been reported in up to 50% of the inherited forms of Hirschsprung disease and in 15-20% of sporadic cases of Hirschsprung disease. The prevalence of Hirschsprung disease in multiple endocrine neoplasia type 2 cases was recently determined to be 7.5% and the cooccurrence of Hirschsprung disease and multiple endocrine neoplasia type 2 has been reported in at least 22 families so far. It was initially thought that Hirschsprung disease could be due to disturbances in apoptosis or due to a tendency of the mutated RET receptor to be retained in the Golgi apparatus. Presently, there is strong evidence favoring the hypothesis that specific inactivating haplotypes play a key role in the fetal development of congenital megacolon/Hirschsprung disease. In the present study, we report the genetic findings in a novel family with multiple endocrine neoplasia type 2: a specific RET haplotype was documented in patients with Hirschsprung disease associated with medullary thyroid carcinoma, but it was absent in patients with only medullary thyroid carcinoma. Despite the limited number of cases, the present data favor the hypothesis that specific haplotypes not linked to RET germline mutations are the genetic causes of Hirschsprung disease.

Primary hyperparathyroidism associated with multiple endocrine neoplasia type I (hyperparathyroidism/multiple endocrine neoplasia type 1) differs in many aspects from sporadic hyperparathyroidism, which is the most frequently occurring form of hyperparathyroidism. Bone mineral density has frequently been studied in sporadic hyperparathyroidism but it has very rarely been examined in cases of hyperparathyroidism/multiple endocrine neoplasia type 1. Cortical bone mineral density in hyperparathyroidism/multiple endocrine neoplasia type 1 cases has only recently been examined, and early, severe and frequent bone mineral losses have been documented at this site. Early bone mineral losses are highly prevalent in the trabecular bone of patients with hyperparathyroidism/multiple endocrine neoplasia type 1. In summary, bone mineral disease in multiple endocrine neoplasia type 1related hyperparathyroidism is an early, frequent and severe disturbance, occurring in both the cortical and trabecular bones. In addition, renal complications secondary to sporadic hyperparathyroidism are often studied, but very little work has been done on this issue in hyperparathyroidism/multiple endocrine neoplasia type 1. It has been recently verified that early, frequent, and severe renal lesions occur in patients with hyperparathyroidism/multiple endocrine neoplasia type 1, which may lead to increased morbidity and mortality. In this article we review the few available studies on bone mineral and renal disturbances in the setting of hyperparathyroidism/multiple endocrine neoplasia type 1. We performed a meta-analysis of the available data on bone mineral and renal disease in cases of multiple endocrine neoplasia type 1-related hyperparathyroidism.

ABSTRACT

Most cases of sporadic primary hyperparathyroidism present disturbances in a single parathyroid gland and the surgery of choice is adenomectomy. Conversely, hyperparathyroidism associated with multiple endocrine neoplasia type 1 (hyperparathyroidism/multiple endocrine neoplasia type 1) is an asynchronic, asymmetrical multiglandular disease and it is surgically approached by either subtotal parathyroidectomy or total parathyroidectomy followed by parathyroid auto-implant to the forearm. In skilful hands, the efficacy of both approaches is similar and both should be complemented by prophylactic thymectomy. In a single academic center, 83 cases of hyperparathyroidism/ multiple endocrine neoplasia type 1 were operated on from 1987 to 2010 and our first surgical choice was total parathyroidectomy followed by parathyroid auto-implant to the non-dominant forearm and, since 1997, associated transcervical thymectomy to prevent thymic carcinoid. Overall, 40% of patients were given calcium replacement (mean intake 1.6 g/day) during the first months after surgery, and this fell to 28% in patients with longer follow-up. These findings indicate that several months may be needed in order to achieve a proper secretion by the parathyroid auto-implant. Hyperparathyroidism recurrence was observed in up to 15% of cases several years after the initial surgery. Thus, long-term follow-up is recommended for such cases. We conclude that, despite a tendency to subtotal parathyroidectomy worldwide, total parathyroidectomy followed by parathyroid auto-implant is a valid surgical option to treat hyperparathyroidism/multiple endocrine neoplasia type 1. Larger comparative systematic studies are needed to define the best surgical approach to hyperparathyroidism/multiple endocrine neoplasia type 1.

We briefly review the surgical approaches to medullary thyroid carcinoma associated with multiple endocrine neoplasia type 2 (medullary thyroid carcinoma/multiple endocrine neoplasia type 2). The recommended surgical approaches are usually based on the age of the affected carrier/patient, tumor staging and the specific rearranged during transfection codon mutation. We have focused mainly on young children with no apparent disease who are carrying a germline rearranged during transfection mutation. Successful management of medullary thyroid carcinoma in these cases depends on early diagnosis and treatment. Total thyroidectomy should be performed before 6 months of age in infants carrying the rearranged during transfection 918 codon mutation, by the age of 3 years in rearranged during transfection 634 mutation carriers, at 5 years of age in carriers with level 3 risk rearranged during transfection mutations, and by the age of 10 years in level 4 risk rearranged during transfection mutations. Patients with thyroid tumor >5 mm detected by ultrasound, and basal calcitonin levels >40 pg/ml, frequently have cervical and upper mediastinal lymph node metastasis. In the latter patients, total thyroidectomy should be complemented by extensive lymph node dissection. Also, we briefly review our data from a large familial medullary thyroid carcinoma genealogy harboring a germline rearranged during transfection Cys620Arg mutation. All 14 screened carriers of the rearranged during transfection Cys620Arg mutation who underwent total thyroidectomy before the age of 12 years presented persistently undetectable serum levels of calcitonin (<2 pg/ml) during the follow-up period of 2-6 years. Although it is recommended that preventive total thyroidectomy in rearranged during transfection codon 620 mutation carriers is performed before the age of 5 years, in this particular family the surgical intervention performed before the age of 12 years led to an apparent biochemical cure.

ABSTRACT

The bone mineral density increments in patients with sporadic primary hyperparathyroidism after parathyroidectomy have been studied by several investigators, but few have investigated this topic in primary hyperparathyroidism associated with multiple endocrine neoplasia type 1. Further, as far as we know, only two studies have consistently evaluated bone mineral density values after parathyroidectomy in cases of primary hyperparathyroidism associated with multiple endocrine neoplasia type 1. Here we revised the impact of parathyroidectomy (particularly total parathyroidectomy followed by autologous parathyroid implant into the forearm) on bone mineral density values in patients with primary hyperparathyroidism associated with multiple endocrine neoplasia type 1. Significant increases in bone mineral density in the lumbar spine and femoral neck values were found, although no short-term (15 months) improvement in bone mineral density at the proximal third of the distal radius was observed. Additionally, short-term and medium-term calcium and parathyroid hormone values after parathyroidectomy in patients with primary hyperparathyroidism associated with multiple endocrine neoplasia type 1 are discussed. In most cases, this surgical approach was able to restore normal calcium/parathyroid hormone levels and ultimately lead to discontinuation of calcium and calcitriol supplementation.

OBJECTIVE: Little information is available on glomerular function changes after surgical treatment of primary hyperparathyroidism. The acute effects of some head and neck operations on renal function were studied. MATERIAL AND MATHODS: Retrospective analysis of changes in creatinine levels and estimated glomerular filtration rate (eGFR) after surgery. Preoperative values were compared with values available until 72 hours after the operation. RESULTS: In tertiary hyperparathyroidism, mean preoperative and postoperative eGFR values were 57.7 mL/min and 40.8 mL/min (p < 0.0001), respectively. A similar decrease was observed after parathyroidectomy for primary hyperparathyroidism, from 85.4 mL/min to 64.3 mL/min (p < 0.0001). After major head and neck procedures, there was a slight increase in eGFR (from 94.3 mL/min to 105.4 mL/min, p = 0.002). CONCLUSION: Parathyroidectomy may be followed by a transient decrease in eGFR that is not often observed in other head and neck operations.


OBJETIVO: Há pouca informação sobre alterações da função glomerular após o tratamento cirúrgico do hiperparatireoidismo primário. O efeito agudo sobre a função renal foi estudado após algumas operações em cirurgia de cabeça e pescoço. MATERIAIS E MÉTODOS: Análise retrospectiva dos níveis de creatinina e da taxa de filtração glomerular estimada (eGFR). Os valores pré-operatórios foram comparados aos valores disponíveis até 72 horas após a operação. RESULTADOS: No hiperparatireoidismo terciário, os valores médios pré e pós-operatórios da eGFR foram 57,7 mL/min e 40,8 mL/min (p < 0,0001), respectivamente. O decréscimo após paratireoidectomia por hiperparatireoidismo primário foi de 85,4 mL/min para 64,3 mL/min (p < 0,0001). Após operações maiores de cabeça e pescoço, houve leve elevação da eGFR (de 94,3 mL/min para 105,4 mL/min, p = 0,002). CONCLUSÕES: A paratireoidectomia pode ser seguida de uma redução transitória na eGFR que não é frequentemente observada após outras operações em cabeça e pescoço.

BACKGROUND: The molecular mechanisms involved in the genesis of the adrenocortical lesions seen in MEN1 syndrome (ACL-MEN1) remain poorly understood; loss of heterozygosity at 11q13 and somatic mutations of MEN1 are not usually found in these lesions. Thus, additional genes must be involved in MEN1 adrenocortical disorders. Overexpression of the glucose-dependent insulinotropic peptide receptor has been shown to promote adrenocortical tumorigenesis in a mice model and has also been associated with ACTH-independent Cushing syndrome in humans. However, to our knowledge, the status of glucose-dependent insulinotropic peptide receptor expression in adrenocortical lesions in MEN1 has not been previously investigated. OBJECTIVE: To evaluate glucose-dependent insulinotropic peptide receptor expression in adrenocortical hyperplasia associated with MEN1 syndrome. MATERIALS/METHODS: Three adrenocortical tissue samples were obtained from patients with previously known MEN1 germline mutations and in whom the presence of a second molecular event (a new MEN1 somatic mutation or an 11q13 loss of heterozygosity) had been excluded. The expression of the glucose-dependent insulinotropic peptide receptor was quantified by qPCR using the DDCT method, and b-actin was used as an endogenous control. RESULTS: The median of glucose-dependent insulinotropic peptide receptor expression in the adrenocortical lesions associated with MEN1 syndrome was 2.6-fold (range 1.2 to 4.8) higher than the normal adrenal controls (p = 0.02). CONCLUSION: The current study represents the first investigation of glucose-dependent insulinotropic peptide receptor expression in adrenocortical lesions without 11q13 loss of heterozygosity in MEN1 syndrome patients. Although we studied a limited number of cases of MEN1 adrenocortical lesions retrospectively, our preliminary data suggest an involvement of glucose-dependent insulinotropic peptide receptor overexpression in the etiology of adrenocortical hyperplasia. New prospective studies will be able to clarify the exact role of the glucose-dependent insulinotropic peptide receptor in the molecular pathogenesis of MEN1 adrenocortical lesions.

Introdução: O hiperparatireoidismo (HPT) é comum na neoplasia endócrina múltipla do tipo I (NEM 1). O tratamento definitivo é cirúrgico. Objetivo: Analisar os resultados da timectomia no tratamento do HPT associado à NEM 1. Método: Estudo retrospectivo de 24 pacientes com NEM 1 submetidos a paratireoidectomia entre 1988 e 2003. Analisaram-se a ocorrência de recidivas e a necessidade de re-operações em pacientes com e sem a timectomia. Resultados: A média etária foi de 37,6 anos, com 12 homens e 12 mulheres. Vinte foram submetidos à primeira cirurgia já com o diagnóstico de NEM1 e quatro pacientes foram operados inicialmente sem esse diagnóstico no pré-operatório (dois em outros Serviços). Nos 20 casos com diagnóstico préoperatório, a calcemia média no pré-operatório que era de 11,5 mg/dL reduziu-se para 8,37 mg/dL nos primeiros dias após a cirurgia e medidas com 6 e 12 meses de pós-operatório; 9,2 e 9,4 mg/dL, respectivamente. Os 13 pacientes com exérese de 4 glândulas: 10 (76,9%) encontravam-se euparatireoideos; 1 paciente (7,7%) apresentou HPT recorrente (retirada de uma glândula intratímica após 10 anos e recidiva do enxerto após 14 anos); e 2 (15,4%) pacientes evoluíram com hipoparatireoidismo. Dos 20 pacientes com diagnóstico pré-operatório, em 17 foi realizada timectomia na primeira cirurgia. Nos 3 pacientes não submetidos a esse procedimento, um apresentou recidiva por glândula intratímica, outro evoluiu com tumor carcinoide tímico e o terceiro está euparatireoideo. Conclusão: O tratamento do HPT na NEM 1 é mais complexo. A realização de timectomia associou-se a melhores resultados no seguimento.


Introduction: Hyperparathyroidism (HPT) is common in type I Multiple Endocrine Neoplasia (MEN 1). Surgery is the definitive treatment. Objective: To analyze the results of thymectomy in the surgical treatment of MEN 1 HPT. Method: Retrospective study of 24 MEN 1 patients submitted to parathyroidectomy between 1988 and 2003. Recurrences and the need of re-operative intervention were evaluated according to the type of operation. Results: Mean age was 37.6 years. Twelve were men. Twenty patients were operated on with the preoperative diagnosis of MEN 1 and four, not (two of them at another facilities). In the 20 cases with preoperative diagnosis, mean preoperative calcemia was 11.5 mg/dL and it dropped to 8.37 mg/dL in the early moment. At six and 12 months means were 9.2 and 9.4 mg/dL, respectively. In 13 patients with four gland excision, 10 (76.9%) were normocalcemic; one patient (7.7%) recurred twice (an intratimic fifth parathyroid after 10 years and autograft recurrence after 14 years); and two (15.4%) presented hypoparathyroidism. Of 20 patients with MEN 1 preoperative diagnosis, thymectomy was performed at the first operation in 17. In the three cases without prophylatic thymectomy, one recurred in a fifht thymic parathyroid, one developed thymic carcinoid and one was normocalcemic. Conclusion: Treatment of HPT in MEN 1 is more complex. Concomitant thymectomy was associated with better results.

INTRODUCCIÓN: Las tasas de mortalidad infantil y neonatal en Argentina tienen tendencia decreciente y las desigualdades entre regiones son notables. El 60% de las muertes en <1 año ocurre en el periodo neonatal, 57% de esas muertes son evitables. OBJETIVO: analizar un conjunto de muertes neonatales a través de técnicas estandarizadas y Análisis de Causa-Raíz (ACR). MÉTODO: estudio multicéntrico observacional de corte transversal. Se incluyeron los recién nacidos (RN) ≥ 500 grs que fallecieron ≤ 30 días entre 06/08 y 03/09 en 6 maternidades. La recolección de datos fue prospectiva multidimensional, próxima a la muerte, a partir de todos los actores, con marcación cronológica y evaluación de dimensiones institucional, recursos humanos, equipamiento, ambiente físico y factores externos. Se caracterizó al RN fallecido y un grupo asesor clasificó la muerte como inevitable o reducible. RESULTADOS: se analizaron 369 muertes; las causas de internación fueron dificultad respiratoria (67,7%), malformaciones (16,2%), infecciones y asfixia perinatal (12%). La dimensión de recursos humanos fue la más frecuentemente relacionada con la muerte (59%), seguida por la institucional y organizacional (48%). La condición de derivación (OR 1,97; IC95% 1,01-3,83) y la presencia de complicaciones (OR 8,56; IC 95% 5,03-14,5) y edad >6 días (OR 2,04; IC95% 1,06-3,95) resultaron asociadas en forma independiente a la reducibilidad de la muerte neonatal. CONCLUSIONES: la mayor parte de las muertes neonatales fueron reducibles. Resulta fundamental establecer estrategias de capacitación y fortalecimiento del recurso humano que asiste a esta población.


BACKGROUND: The infant and neonatal mortality rate in Argentina is decreasing but disparities between regions are notable and 60 per cent of the newborn deaths (NB) are preventable. OBJECTIVE: to analyze a set of neonatal deaths through standardized techniques and root cause analysis (RCA). The RCA is a reactive tool to identify factors contributing to the occurrence of an unwanted event in search of system failures. Design: multicentric observational cross-sectional study. METHOD: NB ≥ 500grs who died ≤ 30 days in 2008–2009 in 6 maternity hospitals in Argentina were included. Data collection was multidimensional and close to the death with chronological analysis and assessment of dimension institutional, human resources, equipment, physical environment and external factors. Every NB was epidemiologically characterized and an advisory group classified death as inevitable or reducible. RESULTS: 369 deaths were analyzed; the main hospitalization causes were respiratory distress (67.7%), malformations (16.2%), infections and perinatal asphyxia (12%). The human resources dimension was the most frequently related to the death (59%), followed by the institutional and organizational (48%). 57.7% of deaths were classified as reducible. The neonatal transport condition (OR 1.97CI95% 1, 01-3, 83), the presence of complications (OR 8,56 CI95% 5,03-14,5) and age > 6 days (OR2 CI95% 1,06-3, 95) were associated with neonatal death reducibility independently. CONCLUSIONS: most of neonatal deaths were reducible. It is essential to establish strategies for training and strengthening human resources that assist this population.
